Summary/abstract:
  • On September 20, 2010, a 55-year-old male career Deputy Chief (DC) responded to a medical call and provided assistance. Later in the day, the DC exercised as part of the Fire Department (FD) fitness program. The DC was about 5 minutes into his exercise program when the duty crew was dispatched to a call in which the DC did not respond. Approximately 90 minutes later, the DC was found unresponsive lying alongside the Stairmaster®. Despite cardiopulmonary resuscitation (CPR) and advanced life support (ALS) at the fire station, in the ambulance, and in the hospital's emergency department (ED), the DC died. The death certificate and the autopsy listed "hypertrophic cardiomyopathy" as the cause of death with "arteriosclerotic coronary artery disease" as a contributing condition. Given the DC's severe underlying heart disease, NIOSH investigators concluded that moderately strenuous physical exertion during exercise probably triggered an arrhythmia causing his sudden cardiac death. NIOSH investigators offer the following recommendations to address general safety and health issues. It is unclear, however, if these recommended programs would have prevented the DC's death: (1) Ensure fire fighters are cleared for return to duty by a healthcare provider knowledgeable about the physical demands of fire fighting, the personal protective equipment used by fire fighters, and the various components of National Fire Protection Association (NFPA) 1582; and (2) Ensure on-duty fire fighters exercise in pairs or within viewing/hearing distance of another crew member.

Summary/abstract:
  • The problem was the Roanoke Fire-EMS Department did not have a process in place to prepare captains to advance to the chief officer ranks even though the department was facing the loss of several chief officers due to retirements. Roanoke Fire-EMS Department captains were not obtaining the education, training and experience they needed in order to prepare themselves for advancement to the chief officer ranks. The purpose of this applied research project was to analyze the level of readiness of the captains and identify areas of needed professional development for captains. Descriptive research was utilized to analyze the readiness of captains for advancement to the chief officer ranks. The following research questions guided the research process: (a) To what extent do the department captains perceive they meet state and national standards and best practices? (b) To what extent do the captains under study demonstrate a willingness to gain the education, training and experience to meet state and national standards? (c) What have other fire departments done to address succession planning? and (d) What can be done to address succession planning in the department under study? The procedures utilized in the research included a review of current literature, a review of industry standards and interviews of department captains. Results show a lack of a succession plan, lack of career development incentives, lack of opportunities to gain new experience and inadequate training and education standards as causing the lack of preparation of captains. Recommendations included establishing a succession plan which included elevating current promotional requirements, providing opportunities for captains to gain experience, developing a mentor program and offering incentives for obtaining higher levels of training, education and experience.
